Requests for copies of Medical Records

Mon Health Medical Center is committed to protecting the privacy of your medical information.  
The following is a step-by-step guide for obtaining medical records from Mon Health Medical Center, Mon Health EMS and the Mon Health Wound Center.

To obtain records from a physician office, please contact that office.

To request medical records from Mon Health Medical Center, Mon EMS, the Wound Healing Center or to request CDs of Radiology or Cath Lab images, you must complete and sign an Authorization for Release of Protected Health Information form.   This form tells us which records to release and where you want them to go, such as to an individual, a physician, medical practice, facility or an agency. 

You may obtain this form at our office, located in the Mon Health Medical Center Main Lobby, in the Registration area.  You may also deliver the completed form to us at this location.  But please understand that there is NO guarantee that we can provide your medical records while you wait.

Cost for Medical Records

The cost of medical records depends on the format of the records, and to whom they are sent or given.

CIOX Health  is a contracted release of information vendor here at Mon Health Medical Center in Health Information Management Services.  

Below are the standard fees for producing a copy of your medical records by CIOX.

Access Fees for PATIENTS ONLY:
  • Electronic records delivered in electronic format $6.50
  • Electronic records delivered in paper $0.90 labor cost to create and deliver the portion of the record maintained electronically plus $0.05 per page for paper and toner 
  • Radiology and Cath Lab CD’s $5.00
  • Plus postage and taxes

*There is no charge for continuity of care if records are sent directly to your physician.   Please allow up to 30 days for processing. 

If you choose to personally pick up your medical records at our office, please be aware that we may NOT be able to provide them to you while you wait.

Mon Health Medical Center and Mon Health employees are not exempt from fees.



MyMonHealth patient portal

As a patient of Mon Health Medical Center, MyMonHealth allows you online access to a portion of your medical records at no charge. This access may also be granted to a parent or child of a patient. MyMonHealth is a private, secure online management tool that provides you with easy access to selected portions of your Mon Health Medical Center health information and to some of our hospital-based care providers.  Learn More about MyMonHealth.


Medical Record Amendment

Every Mon Health Medical Center patient has the opportunity to request an amendment to his or her health information. Please complete the Request for Amendment of the Medical Record and mail it to Mon Health Medical Center, Attn: HIM Director, 1200 JD Anderson Drive, Morgantown, WV 26505. Please understand that your physician may be consulted and Mon Health Medical Center has the option of denying the request if your record is deemed accurate and complete. This process may take up to 60 days from the date the amendment request is received.
